Click here to fill out the registration information and submit it online.	Click here to print out a copy of the registration form. Then complete the information and mail or fax it in.	Please print the form, trim it neatly around the border, and complete one for each quilt entry. Fold the top portion of the form back along the marked line so that your contact information does not show. Using a straight pin, attach the form to the front left edge, halfway up your quilt with “information for judges” facing up. For wall hangings, pin the form at the bottom left of the quilt. Fold your quilt so that the form can be easily viewed for check-in.

Registration forms must be received by 4:00 pm, Monday, April 9, 2012* or until our limit is reached due to space considerations.*
